North Shore Adult Day Health Care

Our rational for providing Nursing Services is to promote, maintain and restore functional level and to prevent deterioration which may possible lead to institutionalization.  Our licensed professional RN’s, LPN’s, CNAs provide the full range of services including but not limited to:

- Skilled observation and assessment of health care conditions. Complete care assessments developed for each member by out multi-disciplinary team

- Wound care: providing for chronic, non-healing wounds secondary to diabetes, chronic circulatory deficiency, pressure ulcers, and wounds as a result of surgical and other medical treatments

- Medication management and teaching

- Bowel/bladder training

- Teach diabetic care and Blood Sugar monitoring

- Foley catheter insertion and teaching care of in-dwelling foley catheter

- Administer vitamin B12, insulin and other IM/Subq. medications

- Teach ostomy or ileo conduit care

- Teach gastrostomy feeding and parenteral nutrition

- Administer inhalation RX

- Monitor vital signs

- Personal care: assist with showering, personal hygiene and activities of daily living

Our rational for providing Recreational Therapy is to maintain physical, mental and emotional wellbeing of the participant. 

Services include: socialization, body awareness, breath awareness, music therapy, horticultural therapy, preventing stiffness in joins, increasing mobility and increasing cognitive capability.

These activities are designed and adopted with patient input in order to be dignified, meaningful and most importantly,  fun.

Participant/Patient council will meet monthly to provide our staff with specific input regarding scheduled activities, which include:

- Daily exercise, music, physical activities and mental stimulation

- Monthly outings to theater, restaurants, museums, parks and fairs

- Adult crafts and games: paintings, ceramics, wood crafts, cooking

- Classes on art, history, music and travel

Our rational for providing Social Services is to improve skills and modify emotional states through individual and group counseling, problem solving therapy, reminiscence therapy, training families and caregivers, case management.  In addition, we act as a community liaison for patients to obtain other community services.  Our social services include:

- Assessments of social and emotional factors impeding health recovery

- Counseling for long-range planning and decision making

- Community resource planning

- Short term therapy

- Stress management and addiction counseling

Our rational for providing Rehabilitation Services is to promote well being, maintain and restore function and mange deficit with activities of daily living.  Services, vary based upon the practitioner’s area of expertise, we employ the following to provide rehabilitative services and consequently

- Physical Therapist provides a full range of services which include: evaluation of condition, gait training, ultrasound, transfer training, home exercise program and prosthetic training

- Occupational Therapist provides evaluation of condition, activities of daily living training, muscle use re-education, perceptual motor training, fine motor coordination, neuro-developmental treatment, sensory treatment, orthotics and splinting and adaptive equipment

- Speech Therapist provides evaluation of condition, voice disorder treatment, speech articulation disorder treatment, dysphagia treatment, language disorder treatment and aural rehabilitation

- Respiratory Therapist provides treatment for respiratory-related challenges

- Dietician provides counsel for healthy eating and proper diet
